Evergreen Health Care Center Receives 5 Star Rating
STAFFORD SPRINGS, CT – Evergreen Health Care Center, which offers long-term and short-term rehabilitation as well as care for those with Alzheimer’s and related forms of dementia, received a 5-star rating from the Centers of Medicare/Medicaid.
There are 229 nursing home and rehabilitation facilities in Connecticut. Fewer than 10% have a 5-star rating. This rating is determined by the outcomes of health inspections, nursing staff care, and nine different physical and clinical quality measures. To receive a 5-star rating from the Centers of Medicare/Medicaid, a nursing home and recovery center must exceed the quality requirements for health inspections, provide comprehensive care to patients individually, and surpass each of the nine quality measures.

The Centers for Medicare/Medicaid launched their rating system to provide residents and their families with an easy way to understand nursing home quality and simplify decisions around finding a high-quality, trustworthy nursing facility.
